One record per shift, built by the people who were there.

Every entry has an author and an attester. A student logs what they saw, a resident who supervised that shift confirms it, and an attending's evaluation attaches to the same rotation.

Student

Log the shift

After the shift, open the Logbook and add one entry per patient: the case, a patient reference, and any procedures performed or observed.

Resident

Attest to it

The supervising resident reviews the batch and accepts it, asks for a modification, or declines it.

Evaluate

Attendings submit a Mini-CEX for an observed encounter, or an End-of-Shift form for the shift overall.

Three groups, one running total — per rotation.

Complaints
11 types
Abdominal pain, chest pain and shortness of breath need two each. Eight others need one.
Procedures
19 types
Bedside ultrasound, EKG & interpretation and Foley insertion need two each. The other sixteen need one.
Evaluations
7 + 7
Seven Mini-CEX and seven End-of-Shift forms, submitted by attendings across the rotation.

Only cases attested and accepted by a resident count toward the total.
